CRITICAL NOTE:
In order for the application to be marked “complete” and sent forward for review, all required application documents must be accurate and complete (see Documents and File Names for Upload for each application type) including:
  1. A list, on appropriate roster, of all students taught (see individual application types for special requirements for selection of preparations and number of students).
  2. CRCT format rosters including FTE numbers for each student listed and all subjects taught (NOTE: Middle school language arts teachers will be evaluated using language arts and reading CRCT results).
  3. Documents are saved using the file names listed in the Documents and File Names for Upload form for each application type.
  4. The application has been approved by the principal and the Local System Review Team (see Application Verification form for required signatures).
Grades 6-8 CRCT
