HIGH ALERT: New application process for Grade 9-12
All Grades 9-12 applicants will submit data for ONE PREPARATION ONLY
 
FOR EXAMPLE:
  • If you teach 3 classes of Algebra II and 2 classes of Calculus, you would complete the required documents for EITHER Algebra II OR Calculus, NOT BOTH.
  • IF you teach content areas that have an End of Course Test (EOCT), you must include one of those content areas as your preparation. In addition, you must complete the EOCT Student Achievement Roster for 2006-2007 in step 5.
Follow all of the steps listed BUT for ONLY ONE PREPARATION! You must include all students that you teach for that preparation on the Monitoring Student Progress in Achievement Roster templates.
 
The Local System Section IV Verification Section now contains this added statement:
 
For Non-State Assessment Rosters and Supporting Evidence:
  • The rosters contain a list of all students taught by the applicant for all classes taught during the school years.
  • Exception: Grades 9-12 submit for one preparation only. The submitted preparation reflects common practice for all classes taught.
Note that the Superintendent, Principal, and two other reviewers are verifying by their signatures.

Step 3: Download and complete: Monitoring Student Progress (MSP) in Achievement Roster for 2006-2007
Step 4: Download and complete: Monitoring Student Progress (MSP) in Achievement Roster template for first semester of 2007-2008
Step 5: Download and complete: EOCT Student Achievement Roster for 2006-2007 IF YOUR PREPARATION CONTENT AREA HAS AN EOCT
Step 6: Download and use the  Student Performance template for the Monitoring Student Progress in Achievement Explanation of Data Template submitted
Step 7: Download and use the  Checklist for Explanation Template to self-assess and/or have a peer assess your application for completeness, accuracy, etc.
Step 8: Access the: Labels for uploads to PSC to:
  • Check for a complete list of documents to be submitted for your grade level and subject areas with brief descriptions of the documents
  • Be sure that the TITLE OF THE LABEL for each submitted document is ON THE DOCUMENT
  • Attach the Labels for uploads to PSC for your grade level and subject areas to your data so that the system operator can clearly identify what is to be submitted. The Labels For Uploads included in the page is a SAMPLE. You must customize it to fit your teaching assignment(s). PSC has limited the number of pages in sample pre-tests, post-tests, and benchmark assessments in Non-Core Curricular data. (Up to 2 pages for elementary, up to 5 pages for middle and secondary schools)
Step 9: Access the Professional Responsibilities document to gather evidence for submission to the Master Teacher Local Management Team for their verification
Step 10: Access the Electronic Data Submission process for guidance in how to submit the application
 
Tips:
I: All supporting documents are submitted electronically by the Local School System Operator. FAXED applications and documents are NOT ACCEPTED
II: SUBMISSION ERRORS: APPLICATION WITH SUBMISSION ERRORS WILL BE PLACED IN THE "CANNOT PROCESS" CATEGORY. PSC STAFF WILL NO LONGER BE ABLE TO CONTACT SYSTEMS/TEACHERS TO CORRECT INFORMATION
